Southern Cross Care (SA, NT & VIC) Inc

A not-for-profit aged care provider supporting 1,400 residents across 17 residential aged care facilities in South Australia and the Northern Territory, including multiple memory support units.

The Challenge

Southern Cross Care supports residents with complex needs across 17 facilities — many living with dementia and unable to self-report pain. Paper-based processes made it difficult to identify pain early, follow up consistently, or give staff the confidence to act quickly at the point of care.

SCC needed a faster, more reliable approach to:

  • Identify pain earlier, including in non-verbal residents
  • Support quick, consistent assessments at the point of care
  • Strengthen follow-up, escalation and accountability
  • Reduce paper-based duplication and fragmented records
  • Integrate with Person Centred Software and medication management systems

The Solution

SCC began with a trial across two memory support units before scaling PainChek® across multiple residential sites. Building on those results, SCC is now expanding its use into home care services.

With PainChek®, SCC now:

  • Assesses pain earlier, more frequently and consistently
  • Strengthens clinical governance and accountability at the point of care
  • Improves staff confidence, particularly when supporting non-verbal residents
  • Delivers better, more measurable outcomes for residents

Key results

  • 68,000+ pain assessments completed: Proactive, consistent pain monitoring without adding workload
  • 72% of assessments scoring zero pain: Greater confidence that pain is being identified, treated and managed
  • 92% of severe pain cases followed up within 1 hour: Timely reassessment, escalation and pain management
  • Improved assessment frequency: Real-time assessments at the point of care
  • More efficient clinical workflows: No more paper-based records and duplication
  • Improved resident quality of life: Better mobility, community engagement and nutrition
